The city  is located in Pike County, Kentucky. It has a population of more than 8,000. The city is a home-rule city; this category includes more than 400 cities, excluding Lexington and Louisville. Pikeville has a long history. It started when state officials agreed to build "Liberty" a new county seat in 1822, close to the Russell Fork River. Due to public disapproval, they decided to change the location to a site donated by a farmer Elijah Adkins. The new town was given the name Town of Pike, but it finally got the name Pikeville in 1850. It was designated an All-American City by the National Civic League in 1965.

The city embarked on one of the most significant projects in the western hemisphere in 1973 to construct the Pikeville Cut-Through, and It was one of the largest civil engineering projects to reduce traffic congestion downtown and eliminate flooding. A massive rock had to be cut, and thousands of rocks and soil were being moved. The success of the project improved the city.

Also, the city has been a center for fast development in the state of Kentucky. The University of Pikeville opened two medical colleges: the Kentucky College of Osteopathic Medicine and the Kentucky College of Optometry in 1997 and 2016, respectively. In addition, the Medical Center in the community attained the status of a regional medical facility. This led to an expansion of the facility in 2014 to include an 11 story clinic and ten-story parking lot.
